Making an Appointment

Request an appointment for a new or ongoing health problem

All you have to do is answer a few simple questions and we will get you the help you need. One of our Clinicians will review the information and decide next best steps. This might mean booking an appointment with a Clinician or guiding you to another helpful service. We are here to make sure you get the care you need.  

How do I request an appointment?

3- Telephone or call into the surgery and we will complete the request for you

Your answers are sent to your GP who responds as quickly as possible during our opening hours. Initial responses may be via online message or telephone and face- to-face or video consultation appointments will be arranged if needed.

And if you need any help please pop into the surgery and we can help you get set up.

What does this mean for you as a patient?

– You do not need to wait on the telephone lines to have your query answered. 

– Patients can send a non medical request through 24 hrs a day. 

– You can order a repeat med3 (Sick note) certificate online.

– You can explain your symptoms clearly by adding a description, which will be assessed by a Clinician.

– You will be sign posted to the most appropriate service by a clinician, including same day/future appointments or a more appropriate service.

If you have a medical emergency or a life threatening condition please DO NOT use this system.

Non-urgent advice: Cancel an Appointment

If you cannot attend an appointment for any reason please inform us as soon as possible in order for us to give the slot to someone else.

Did Not Attend (DNA) Register

Every month hundreds of appointments are wasted because people book them but then fail to attend without cancelling. Sometimes even on the same day of booking. This means they are not available for anyone else to use and they have to wait longer to see the Doctor, Paramedic Practitioner, Physician Associate, Nurse etc.

Remember, the previous working day could be before a weekend or bank holiday.

To try and stop this happening we have a DNA register for patients who did not attend a surgery appointment on at least 3 occasions in the last 3 months.

If you are on the DNA register you can book appointments in the usual way but MUST either cancel or confirm in advance that you will be attending. If you do not confirm, the appointment will be cancelled and made available for another patient to use. If you then come in for your appointment, you will not be seen, but will be asked to rebook for another time.

When you book your appointment we will remind you of the need to confirm and you must confirm the day before.

It’s easy to CONFIRM or CANCEL – simply phone 01253 623896 option 1 and again option 1. or E-mail the surgery at [email protected] giving your full name DOB and date and time of your appointment. You will remain on the DNA register until you have completed 6 months of full attendance.

Appointments can also be cancelled by replying CANCEL to your appointment reminder message or by using online services.

Evening and weekend appointments

Evening and weekend appointments are available with GPs, practice nurses and other healthcare professionals for routine pre-booked services.

The Fylde Coast NHS is committed to improving access to primary care services. This includes making appointments available at times that are convenient to patients.

If you:

  • find it difficult to attend an appointment during the working day;
  • are a busy parent with small children;
  • or rely on working carers to take you to appointments;

then this service is here to support you.

About the service

The Fylde Coast NHS has arranged for additional appointments to be provided outside of normal GP practice hours by the Fylde Coast Integrated Urgent Care Service.

The service offers pre-bookable and same-day routine primary care appointments with a range of clinicians including GPs, nurses and health care assistants.

Please be aware that this is not a walk-in service – you must have an appointment.

The service employs local clinicians from the Fylde Coast, where possible.

Appointments

An appointment must have been booked in advance of visiting the service.

When your GP practice is open
To book an appointment, contact your GP practice.

When your GP practice is closed
Appointments may also be booked in Blackpool by calling 01253 952950.

Appointments are for pre-bookable and same-day routine and not for urgent care. If urgent care is required, please call NHS111 for advice.

When you book an appointment the practice will ask you for consent to share your detailed GP record with the clinician providing the care.

Should you need to cancel your appointment please contact your GP practice. If your GP practice is closed, please call 01253 952950 to cancel.

Home Visits

It is better to be seen at the Medical Centre where we have appropriate examination and treatment facilities, and where you can be seen more quickly.

However, if you are too ill or frail to come in, please contact us before 10:00am to request a home visit.

You will be asked for brief details of your problem so that urgent cases may be seen first.

The doctor may contact you by phone before visiting.

Please be aware that home visits by nurses (including district nurses), such as for blood tests, flu vaccinations, dressings or annual reviews, are only for patients who are genuinely housebound and cannot be brought to the surgery.

Late For Your Appointment

Please attend your appointment on time, if you are late you may not be seen. If you are not seen you will not be able to rearrange your appointment until the next working day-except in the event of an medical emergency that requires immediate attention.

Text Reminder Service

We have a texting service which allows you to receive confirmation and reminders about your appointments.

To have this service you will need to register by completing a consent form.

Please remember to update your contact details with us when you change address, telephone numbers and email address.
